Why these notes are usable

The attendance entry describes the arrival without speculating about the reason. The props request includes a scene, observed problem, owner and deadline. Lighting's note distinguishes a request from an approved design. The next call includes the people and planned work, not just a time.

Adapt it to your production

Use your own department names, agreed attendance categories and distribution list. Add more break rows when needed. Record follow-up completion in the next report or the production's action tracker. Do not copy the sample people or dates into an issued report.
